Rising 6th Grade Students and Parents
A booster dose of the Tetanus, Diphtheria, Pertussis (Tdap) vaccine is required for all students entering the 6th grade. Students that are 10 years old and above can receive this vaccine NOW.

Please do not wait until the end of the summer! Tdap boosters may be obtained from your doctor, military clinics, or The Prince William County Health Department.

Upon receiving the vaccination, bring the written documentation to the Gainesville Middle School Counseling Center during the summer.

Student schedules will not be provided in August to students that do not show proof of their vaccination, so please plan accordingly!

Medication, Illness and Immunizations

I· If you have any medical concerns regarding your child, please email, call, or stop in to see me.

· Medical/medication forms must be completed EVERY school year. Links to forms are located on the right side bar OR all PWCS Health Services forms can be downloaded from the following link:
http://studentservices.departments.pwcs.edu/modules/cms/pages.phtml?pageid=167108&sessionid=f0ae1fc52c413cf12e72c45ff752f8ec

· Students are not allowed to carry over the counter (OTC) or prescription medication. (Exceptions to this rule are asthma inhalers and epi pens with proper documentation.)

o Any OTC medication must be new and in an unopened container and brought to the clinic by the parent.

o Prescription medication must be brought to the clinic by the parent in the original container with the prescription on it and proper documentation signed by the doctor.

· Parents may bring in one bottle of OTC medication (ex. Tylenol etc.) to be shared by siblings. Due to space issues, please do not bring in Costco size bottles.

· Following state guidelines, it is important for the well-being of our students and staff that all immunizations be kept up to date. Noncompliance of these state guidelines will lead to the exclusion of the student from school.

By working together, we can keep our students healthy and productive at school.
